Delivers basic nursing care, including activities of daily living (ADLs) and restorative program(s) based on individual patient needs and plan of care.

Ensure compliance with company policy as well as federal, state, and local regulations.

Thoroughly document all care provided, patient response to care, and patient refusals of care.

Accurately measures and documents vital signs, height, weight, and other necessary patient data.

Takes ownership of patient hydration rounds to ensure all patients maintain adequate hydration.

Provide meals per patient preference (in-room/dining room); assist with meals as needed based on individual patient care plan and monitor / document patient intake.

Respond to call bells, signal lights, intercoms, and patient requests for assistance in a timely manner.

Immediately communicate changes in patient condition to licensed nurse.

Utilizes appropriate individualized behavior response techniques under the direction of the licensed nurse.

Safely transfers and transports patients using proper body mechanics and appropriate safety equipment.

Operates all equipment / devices according to guidelines / operational instructions.

Complete frequent room rounds to ensure clean, orderly, and safe patient environment.

Participates in the admission / discharge / transfer process.
