Responsibilities and Essential Job Functions
Patient Assessment: Evaluate patients' medical history, dietary habits, and lifestyle to determine their nutritional needs.Personalized Meal Planning: Create and implement individualized meal plans tailored to patients’ specific diabetes management goals.Education and Counseling: Teach patients about the relationship between diet and diabetes, offering practical advice and strategies for healthy eating in one-on-one and group settings.Monitoring and Evaluation: Track patients' progress, adjust meal plans as necessary, and provide ongoing support and encouragement.Collaboration: Work with healthcare providers, including providers and nurses, to ensure a cohesive approach to diabetes management.Resource Development: Develop and utilize educational materials and resources to aid patients in understanding and managing their condition. Participate in community events, as appropriate.Competencies: Perform and maintain the professional, clinical, and technical competencies required within the department.Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities.
