Overview Ensure access to care by expertly navigating Medicaid eligibility and application processes, you will help patients secure the financial support they need for skilled nursing and long-term care services. Support efficient facility operations through accurate and timely processing of Medicaid verification, admission screening, and financial assessments, you will streamline workflows, reducing delays in-patient admissions. Perform intake function including clinical and financial review of all referrals as designated by assignment. This includes insurance benefit verification, approval or escalation based on clinical and financial capabilities, and collaboration with multiple referral sources. How you Will make a difference Conduct Medicaid admission screenings by performing PAE/Level of Care screens, asset checks, and financial verifications to assess eligibility for long-term care services. Collaborate with teams & escalate issues by working with clinical, financial, and intake teams to ensure all necessary documentation is complete and escalate concerns, such as financial “red flags” or lack of legal representation, to leadership when needed. Guide patients & families through the Medicaid process by providing education and assistance in completing pre-screening applications and addressing eligibility concerns. Maintain a working knowledge of Federal and State regulations, referral processes, and reimbursement (Managed Care, Medicare, and Medicaid). Prepare all benefits verification documentation and clinical summaries to appropriate facility to ensure a timely admission. Manage the Customer Relations Management System by entering leads from referrals to closing. Collaborate regularly and maintain open communication with SHC leadership, SHC stakeholders, potential residents and families, referral sources. What you Need to make a Difference Minimum of two (2) years’ experience in long-term care or related industry. Minimum of two (2) years’ admission experience; referral management experience preferred. Knowledge of Medicaid eligibility requirements and long-term care application process for all SHC markets Must understand long-term care business, including census and quality mix goals and referral expectations. Effective verbal and written English communication skills. Demonstrated intermediate to advanced skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation. Highest level of professionalism with the ability to maintain confidentiality. Ability to communicate at all levels of organization and work well within a team environment in support of company objectives. Customer service oriented with the ability to work well under pressure. Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
