The position is Responsible for coordinating and implementing post-discharge plans in coordination with the Case Managers through the use of Extended Care Information Network (ECIN). They are also responsible for assisting with advocacy and referrals to other community resources.

Job Requirements

Graduate of Accredited Master's in Social Work ProgramLicensed Clinical Social WorkerKnowledge of community resources used for discharge planning, hospital operations, excellent communication/presentation skills, knowledge of third party payment systems, Medicare/Medicaid programs.Maintains current knowledge base of community services through continuing education.Ability to multi-task, set priorities and maintain organization.Computer skills.

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly

Founded in 1976, Holy Cross Hospital - Davis has grown into a 220-bed hospital of choice for the flourishing communities of northern Utah.
