Assist with problem solving, attaining goals and provide information throughout hospitalization for our patients. Human Resource Contact: Brenda Hales /brenda.hales@wyo.gov/ 307-444-0791 ESSENTIAL FUNCTIONS: The listed functions are illustrative only and are not intended to describe every function which may be performed in the job level. + Responsible for individualized case planning, discharge planning, and monitoring progress. + Responsible for case management duties to include planning and implementing all facets of programming and associated documentation. + Prepares, organizes, schedules, and implements psychoeducational groups and interventions. + Monitors patient compliance with established goals and objectives; updates case plans as needed. Qualifications PREFERENCES: Preference will be given to qualified applicants who are currently a certified Social Worker. KNOWLEDGE: + Knowledge of human behavior, motivation, and interpersonal behavior. + Knowledge of the varied techniques of programming/education. + Knowledge of social work principles and practices. + Knowledge of interviewing and counseling techniques. + Knowledge of current social, economic, and health problems and community resources. MINIMUM QUALIFICATIONS: Education: Bachelor's Degree (typically in Social Services) Experience: 0-2 years of progressive work experience (typically in Casework) OR Education & Experience Substitution: 3-5 years of progressive work experience (typically in Casework) Certificates, Licenses, Registrations: None Necessary Special Requirements PHYSICAL WORKING CONDITIONS: + Physical ability to lift, twist, bend, pull, and push up to 50 lbs. NOTES: + FLSA: Non-Exempt + Pre-employment drug and alcohol testing will be conducted on safety-sensitive positions.
