What you will do:

The Alzheimer's Case Worker provides service and care coordination to clients and their families in the community living with Alzheimer's disease.  Work involves coordinating respite services based on the client’s needs, which will include in-home care, adult day programs, and long-term care facilities.  Additional responsibilities will include receiving and submitting bills for payment and maintaining contracts with current providers.

The Alzheimer's Case Worker will work with the rest of the Eddy Alzheimer’s Services team to ensure appropriate and person-centered services are provided through the use of high-level creative problem solving and advanced communication skills.   

Requirements:

Previous knowledge of Alzheimer’s disease and experience working with the Aging population preferred.Bachelor’s Degree required.

*This position is grant funded and employment is contingent on continuation of grant funding.

Pay Range: $23.40 - $33.69

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
