We’re seeking a compassionate Case Manager to join our Substance Use Disorders team. In this role, you’ll deliver community-based support by meeting individuals where they are, including their homes and other local settings, to coordinate care and connect them to essential services.  

 

Earn $22.38 per hour plus benefits, or $24.38 per hour with the selection of enhanced pay and a scaled back benefits package.

Position Details: 

This is a full-time, community-based position with travel throughout Delaware County, PA. Mileage reimbursement provided.  Support a caseload of adults with Mental Health and Substance Use Disorders.  Typical schedule of Monday – Friday day hours.  Deliver mobile case management and care coordination in clients' homes and community settings. Create and maintain recovery-focused Individual Service Plans (ISPs).  Identify and connect individuals to appropriate housing, employment, and healthcare resources.
