The Case Manager facilitates the treatment components of EISS. Provides direct practice in the areas of group work, consumer education, treatment planning, and individual counseling as needed.

Facilitate treatment components of EISS Provide direct practice in the areas of group work, consumer education, treatment planning and individual counseling as neededParticipate as a member of the multidisciplinary treatment team, performing intake assessments, initiating treatment interventions and crisis intervention Perform clinical case management and treatment planning with consumers that experience psychiatric disorders and substance abuse disordersAll clinical documentation is completed in compliance with the requirements set by the NJ Division of Mental Health Services and the Joint Commission on Accreditation of Health Care Organizations.Participates in staff training and continuing education. Takes an active role in Performance Improvement projects at EISS and MMC Attends and participates in multidisciplinary staff meetings daily and is prepared to discuss clinical and program issues.Facilitate Patient Education through individual counseling and group work. Prepare education materials fordistribution and presentation to mental health consumersAssists in patient billing through accurate record keeping of client's attendance in groups and activities dailyAssume active participation in Performance Improvement Projects. Initiate improvement efforts and work collaboratively with the treatment team to identify areas of needed improvement.Attend Inservice training to improve your skill level and knowledge base. Participate in the training preparation and planning process when required.Support Department in service and continuing education committee,Complete Treatment Plan with direct participation from each consumer on caseload. Attend treatmentPlanning meetings and conducting treatment strategies with other members of the treatment teamComplete documentation in the medical record according to department policy Psychosocial AssessmentsGroup Notes Progress Notes and treatment summaries include all required information and meet the time frame requirements according to policyFacilitate Family Psycho-educational Program,Facilitate Family Psychoeducational Program Provide information and support group for family members of adult partial hospital program clientsFacilitate Patient Education through individual counseling and group work Prepare education materials for distribution and presentation to mental health consumersFacilitate Treatment Groups as scheduled Lead mutual aid support groups and skills training activitiesPerform group work techniques and interventions to achieve individual goals for each group memberFacilitate individual and Family Counseling according to treatment plan recommendationsFacilitate referrals to other services agencies and assist clients in obtaining appropriate entitlementsSocial Security Benefits Residential program Self HelpFunction as liaison with inpatient unit specifically completing assigned consults within 48 hours and attending alternate treatment team meetings with the inpatient unit on Tuesday morningsParticipate in the departments QAPI efforts Complete QAPI monitors and follow-up on action plansProvide Recommendations for improvement effortsPerform Intake Assessments following initial contact or referral Present Assessment to treatment team and complete the required documentation for clinical recordPerform crisis Intervention with the cooperation of other treatment team members,Perform interventions with the Screening center Staff when requiredPerforms intakes on assigned referrals Complete necessary paperwork in 2 week time frameProvides supervision for program socials and therapeutic recreationBenefits and Perks:
