Case Manager Float - Full-Time

*Full-time (M-F 8am-4:30pm)*

 

Experience in physical medicine and rehabilitation strongly preferred.

 

This Case Manager will cover the locations in the western St. Louis region - Warrenton, Lake St. Louis, St. Charles, Chesterfield and Bridgeton.  In addition, there may be times when coverage of Olivette, Kirkwood, Arnold and Florissant will be needed.

 

This position includes mileage reimbursement.

Responsibilities

Under general direction monitors and assesses patient plan of care from pre-admission to post- discharge.

As part of the treatment team assists patient/family in goal setting to maximize patient’s independence in both home and community. Interacts with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.

Qualifications

Professional Healthcare Degree in either 

Nursing Social Work Licensed PT, OT, SLP 

One year medical rehab case management experience (preferred)
