Case Manager - Fairport Creekside BOCES
Job Summary The Case Manager – Community Based services low to medium acuity adults and youth in their communities as a member of the Care Team with the goal of reducing and preventing emergency room visits, hospitalizations, and improving overall functioning. This position utilizes assessments, linkage to services and community resources, advocacy, and support to coordinate services for adults, youth, and families using person centered/family driven care strategies. This is a Monday-Friday, Full-Time position, on a 7:30am-4:30PM schedule. Essential Job Functions + Partner with referral sources to engage family and build interdisciplinary care team to support the family in meeting their needs. + Manages the intake process by reading the referred materials, written intake reports, completing all releases, requesting additional information as needed and follow up with the family. + Conduct initial and ongoing assessments to assist with accomplishing client goals and needs within program requirements. + Facilitates groups/trainings/events that provide education, support, and community building for families and youth. + Provides, when appropriate, transportation for youth to and from services and other community services to facilitate youth’s ability to access services. + Refer members to applicable agency services and community programs, such as outpatient counseling, dental provider, and primary care providers. + Support members who transition between systems and services (i.e. hospitalizations, inpatient stays, residential settings, housing needs, etc.). + Establishes and maintains productive working relationships with community service providers to facilitate effective referrals and service evaluation. + Maintain required contact with participants, their families, and the interdisciplinary team and faci litate team meetings. + Serve as a liaison between the program and other internal and external resources, ensuring information is shared with the Care Team + While this job description covers many aspects of the role, employees may be required to perform other duties as assigned. Education & Experience + Bachelor’s degree in Human Services or a related field required + Minimum 2 years of relevant experience required OR + Master’s degree in Human Services or related field and minimum 1 year of relevant experience required Special Requirements + Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards. + CPR certification or equivalent required (obtained within 60 days of hire) Knowledge, Skills & Abilities In addition to demonstrating the Hillside Professional Competencies of Communicates Effectively, Personal Excellence, Cultural Competence, Builds and Leverages Relationships, and Optimizes Decision Making, the following occupational competencies must be demonstrated: + Strong organization skills (i.e. ability to manage several different tasks and large caseloads) + Time management skills (i.e. ability to manage scheduling visits with a large caseload and complete tasks by funder deadlines) + Willingness to travel + Strong communication skills (i.e. able to manage having difficult discussions with others) + Ability to navigate different systems + Ability to find resources in different communities + Ability to learn and utilize an electronic medical record system for all case record documentation requirements Physical Demands & Work Environment + Must be able to work a variable scheduling including evenings and weekends + The following daily physical demands are required: + Sitting (up to 6 hours) + Occasional standing (up to 2 hour) + Occasional walking (up to 2.5 hours) + Driving (several times a week up to 6 hours) + Continuous balancing (up to 8 hours) + Occasional balancing, bending, stooping, climbing, kneeling, pushing, pulling, reaching forward or down, reaching overhead, running, and twisting (up to 2.5 hours) + Weekly lifting up to 10-15 lbs. on a daily basis + Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.) + Occasional exposure to dust, fumes, gases, chemicals, or smoke is apparent + Ability to change positions as needed SPECIAL CONSIDERATIONS While Hillside is a restraint free environment, there may be times in a “life or limb” situation, that staff may be required to physically restrain clients weighing between 50 lbs. and 300 lbs. and guide them safely to the floor. In some circumstances, restraints can last up to 15 minutes and may require repetition as necessary. $21.40 Minimum pay rate, $31.00 Maximum pay rate, based on experience.
