Job Summary: Case Management Extenders role is to support varies functions of Case Management that can be performed by a nonlicensed care team member. They are able to assist the patient and follow the patients individualized plan of care with the guidance of a professional case manager.
Provides administrative support to Case Management teams as needed to support workflow process and outcomes to include: answer multiple and designated telephone lines, prioritize and route calls, demonstrates effective and interpersonal communication skills, and utilizes computer skills at an intermediate level. Collaborates with Case Manager / Social Worker to provide post-acute referral for discharge needs. Maintains and expands awareness of post-acute care settings such as Skilled Nursing Facilities, Home Care Agencies, Infusion Services, Acute Rehab and Long Term Acute Care Facilities. Coordinates patient transport when requested as well as medication assistance programs. Assists with payor communications to care team and records these into the patient’s medical record. Assist with front office duties as needed. Enters referral notes, status of bed offers, and other details as necessary. Independently and proactively initiates the follow-up with post-acute referral setting to ensure a timely discharge. Provides patient notices and patient choice timely and with explanation to patient and families based on mandated guidelines set forth by Medicare conditions of participation. Performs all other duties as assigned.
