Job Overview:
Responsible for providing comprehensive medical case management, health, and psychosocial education to CCC patients, families and significant others.
Qualifications:
Required:
Bachelor Degree required in Social Work or Social Work related field. Demonstrates knowledge of case management standards. Demonstrates the ability to function as a member of an interdisciplinary team.Preferred:
Master’s Degree in a mental health degree that terminates in licensure strongly preferredExperience and knowledge of HIV, entitlement programs, and community resources is preferredEssential Functions:
Conducts risk reduction, adherence counseling, and provides referral for services to patient’s with the goal of preventing HIV transmission.Provides counseling and information regarding advance directives, and assists in the completion of forms. Refer and link patient’s to appropriate services that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Maintains up to date records of all patient encounters and necessary documentation in the chart. Monitor and complete timely documentation to ensure that follow- up of all referrals is made.Provide access to emergency financial assistance (e.g. food vouchers, utility payment assistance, and transportation) as needed to promote and maintain positive health outcomes.Provide supportive counseling as needed and refer appropriate patients to the mental health counselorMaintains HIPPA guidelines and confidentiality when interacting with patients, families, personnel and the general public.Responds in a timely manner to verbal and written requests from patients, inter-disciplinary team members, and administration, medical and public agencies.Attends scheduled medical case management trainings.Attends scheduled medical case management meetings.Complies with unit and departmental scheduling guidelines.Promotes and fosters a positive work environment.
