Conduct comprehensive assessments of patients' medical, social, and psychological needs + Collaborate with the treatment team to develop individualized care plans. + Coordinate and monitor the delivery of services, including therapy, medical care, and support services + Advocate for patients' rights and ensure their needs are met throughout their rehabilitation journey + Provide education and support to patients and their families Qualifications What you will need: + Bachelor's or Master's degree in a related field preferred (e.g., Social Work, Nursing, Psychology) + Clinical case management experience, preferably in a rehabilitation or healthcare setting + Strong interpersonal and communication skills + Knowledge of brain injuries and rehabilitation preferred About our Line of Business ResCare Community Living, an affiliate of BrightSpring Health Services, has five decades of experience in the disability services field, providing support to individuals who need assistance with daily living due to an intellectual, developmental, or cognitive disability.
