Case Manager

Summary:

 

The Case Manager is responsible for maintaining a caseload of both pre-release and post release clients in the RISE department for women reentering the community.  The basic responsibilities include: intakes, assessments, monitoring their daily activities, assisting them with employment, housing, transitional issues, and other elements to assist them in a successful reentry in the community. The role is entirely in person.

 

Duties/Responsibilities:

Handles the initial intake to the program and assessment as well as develops and monitors an Individual Program Plan for clients on their caseload. Meets regularly with each resident to discuss progress with program requirements, personal goals, program compliance, review of finances (wages, subsistence payment, budget, savings, and any financial obligations) and discusses, reviews and helps with meeting their treatment goals. Communicates with contracting and other relevant agencies regarding various aspects of Individual Program Plans. Maintains ongoing communication with service providers and family members, and Coordinator and Director as relevant to the continued progress of each resident on caseload. Provides resources and referrals for external agencies/professionals based on the client’s treatment needs. Provides and stocks closet for basic needs for clients. Provides timely information to appropriate personnel, funders, referral source. Completes all required paperwork for each resident on caseload. Facilitates the support groups. Attends necessary meetings, conferences, and training within CRJ and with outside providers as needed and required.  Group/Individual client contact at women’s prison. Other responsibilities as assigned by the Coordinator/Director.

Case Manager Minimum Qualifications:

BS/BA from an accredited college or university in Criminal Justice, Human Service, or related field.  Prefer one year of case management experience, prefer experience with a correctional population, in a residential setting. Ability to speak Spanish is a plus. Must have own transportation and a valid driver’s license and willingness to transport clients when needed.
