Description We are looking for a dedicated Case Manager to join our team in Memphis, Tennessee for a non-profit. In this Contract-to-permanent role, you will work closely with community members in need, helping them secure housing and navigate essential resources. This position offers an opportunity to make a meaningful impact while leveraging your organizational and interpersonal skills. Responsibilities: • Collaborate with clients to identify housing solutions and connect them with relevant resources. • Maintain detailed records of client demographics and progress using tracking databases. • Utilize spreadsheets and Microsoft Excel to organize and analyze data effectively. • Foster strong relationships with clients, housing facilites, and community organizations. • Conduct regular check-ins with clients to ensure ongoing support and address any challenges. • Monitor and document case progress to ensure compliance with program guidelines. • Provide guidance and advocacy for clients as they navigate housing and social services. • Coordinate with team members to develop and implement strategies for improving client outcomes. • Assist with property management tasks, including communication and coordination with landlords. Requirements • Proven experience in social work, or a related field. • Proficiency in Microsoft Excel and data entry tools. • Strong organizational skills and attention to detail. • Ability to establish and nurture relationships with diverse populations. • Familiarity with tracking systems and database management. • Excellent communication and interpersonal skills. • Knowledge of housing programs and resources is highly desirable. • Ability to work independently and as part of a team.
