You will ensure high-quality, efficient outcomes by collaborating with healthcare teams, patients, families, and payors. This role requires leadership in communication and problem-solving to meet clinical and organizational goals. **What you will do:** + Lead team meetings and coordinate care plans, resolving treatment discrepancies + Monitor patient progress and adjust care plans to optimize outcomes + Ensure compliance with laws, safety regulations, and organizational values + Facilitate timely discharge planning across the continuum of care + Manage resources effectively, focusing on cost, quality, and length of stay **What you will need:** + BSN preferred with RN Indiana licensure required + 3-5 years clinical nursing experience, critical care highly desirable + Certification in case management preferred + Strong critical thinking and communication skills + Computer proficiency; Spanish language skills a plus **Our Commitment** Rooted in our Mission and Core Values, we honor the dignity of every person and recognize the unique perspectives, experiences, and talents each colleague brings.
