Case Manager
Job Title: Case Manager
Job Description

This position is based in a community clinic and focuses on case management. The role involves completing initial and 180-day assessments for participants and being highly involved in hospital discharges. You will work closely with hospital discharge planners to ensure safe and appropriate discharge plans and communicate recommendations effectively. Additionally, you will coordinate skilled nursing facility discharges for participants returning to the community or requiring rehabilitation or skilled needs. You will be part of an interdisciplinary team (IDT) to ensure continuity of care for the participants you oversee.

ResponsibilitiesComplete initial and 180-day assessments for participants.Assist and communicate with hospital discharge planners for safe and appropriate discharge plans.Coordinate skilled nursing facility discharges for participants.Ensure continuity of care by working within an interdisciplinary team.Essential Skills1+ years of experience in Case Management.Proficiency in case management, social work, discharge processes, and EPIC.Familiarity with utilization review, appeals, care coordination, and discharge planning.MSW in WA State.Additional Skills & QualificationsBasic Life Support (BLS) certification.Licensed Clinical Social Worker (LCSW) preferred.Work Environment

You will work in an outpatient facility, providing services and support in a community clinic setting.

Pay and Benefits

The pay range for this position is $40.00 - $55.00/hr.
