**Job Type:** Regular **Time Type:** Full time **Work Shift:** Day (United States of America) **FLSA Status:** Non-Exempt **When you join the growing BILH team, you're not just taking a job, you’re making a difference in people’s lives.** WE CARE by fostering the wellbeing of a health-focused workplace that supports work-life balance, empathy in understanding others' perspectives, collaboration for extraordinary results, accountability to achieve collective goals, respect for diversity and inclusiveness, and equity by ensuring everyone has the opportunity to reach their full potential. **Job Description:** _Essential Role Responsibilities:_ _Case Management Support Functions_ _Demonstrates knowledge and understanding of the electronic SNF referral process._ _Demonstrates ability to retrieve and print from Epic all clinical information needed for the SNF referral packet._ _Demonstrates ability to evaluate a CM SNF referral and request additional information if indicated utilizing navihealth, our electronic referral program._ _Demonstrates collaboration and cooperation with SNF’s and acute rehab facilities during the referral/screening process._ _Demonstrates collaboration and cooperation with the Case Managers and Social Workers during the SNF/acute rehab referral screening process._ _Executes the SNF/acute rehab packet without delay once referral is received._ _Assists Case Management staff on the nursing unit with the assembling of the discharge envelope._ _Demonstrates skills and ability to plan for and facilitate procurement of necessary durable medical equipment, supplies and resources to promote successful patient discharge outcomes._ _Communicates with commercial payors to check the status of authorizations for post-acute placement._ _Information/Office Management_ _Demonstrates skill in data entry and production of documents._ _Demonstrates knowledge and skill for information management and retrieval._ _Epic, Navihealth, MS Word and Excel proficiency._ _Demonstrates ability to provide telephone coverage for the department._ _Assists with other general data entry, retrieval and clinical duties as duties as assigned._ **As a health care organization, we have a responsibility to do everything in our power to care for and protect our patients, our colleagues and our communities.
