Conduct intake assessments with individuals seeking housing assistance to determine needs and eligibility.
Develop individualized service plans outlining housing goals and supportive services.
Maintain and manage a caseload of clients, providing ongoing support and case management.
Communicate regularly with clients, primarily via phone, to monitor progress and provide guidance.
Submit monthly reports documenting client progress, service delivery, and compliance with program requirements.
Process and submit payment requests for housing-related expenses (e.g., rent, hotel stays).
Coordinate with landlords, hotels, and housing providers to secure and maintain housing placements.
Identify and address barriers to housing stability, such as employment, childcare, or transportation needs.
Update client records and case notes in internal systems to ensure accurate documentation.
Track and report client participation in work activities and notify appropriate agencies of non-compliance.
Stay informed about local housing resources, labor market trends, and community support services.
Maintain regular contact with clients placed in employment or training programs.
Complete all required documentation and forms for county and program compliance.
Collaborate with employment organizations, educational institutions, and other service providers.
