Job Description
The role of a Case Management Assistant involves assisting with UR transmission, managing a high volume of faxes and voicemails, and performing data entry tasks. You will coordinate and implement the transition of care and discharge plans for ambulatory patients, ensuring timely discharge through critical thinking, teamwork, and effective communication between care providers, patients, families, and external vendors.
ResponsibilitiesCollaborate with Case Managers and Social Workers in baseline patient assessments to identify post-hospital support and discharge needs.Communicate and problem-solve with Case Managers and Social Workers to develop the transition of care plan, offering choices for post-acute providers and sharing expected discharge dates.Ensure the patient and medical facility receive information on benefit coverage, partnering with payers when needed.Monitor progress towards meeting transition of care goals and escalate barriers to Case Managers and Social Workers.Keep the patient and medical facility informed of the progression of the transition of care plan throughout the hospital stay.Coordinate all necessary post-discharge referrals and authorizations in collaboration with the care coordination team.Facilitate the transfer of a patient to an appropriate post-acute facility by preparing documents for the receiving provider and assisting with transportation services.Round with Case Managers and Social Workers on units to provide updates and receive direction on assistance needed.Deliver the Medicare 'Important Message' and inform the patient or medical facility of their right to appeal the discharge.Participate actively in daily team huddles and care coordination department meetings.Maintain accurate, current, and legible documentation according to department standards.Provide clerical support as needed, including copying, faxing, scanning, and data entry.Comply with confidentiality policies, HIPAA regulations, and department standards when transmitting patient information.Essential SkillsExcellent oral and written communication skills.Strong interpersonal and time management skills.Ability to work effectively in a fast-paced environment with rapidly shifting priorities.Ability to work independently with minimal direction.Intermediate PC skills and word processing skills.EPIC experience (required).Knowledge of payers and case management experience preferred.Ability to type 55 or more WPM.Additional Skills & QualificationsHigh school diploma or equivalent.Must have health insurance knowledge.Must have 1 year of recent healthcare experience within inpatient or outpatient settings.EPIC is highly preferred but not required.Adaptability to change.Strong teamwork and conflict management skills.Work Environment
This is a fully remote position offering the opportunity to gain experience working for a large hospital system from home. Candidates must be able to work rotating weekends and from 10am-6:30pm PST, regardless of their location. This role offers the potential for a long-term contract with consultants having been on contract for over a year.
Pay and Benefits
The pay range for this position is $18.00 - $18.00/hr.
