Work hours: Full time; Tuesday - Saturday

Here's What You Need
* H.S. Diploma or Equivalent required
* Three years' relevant experience, preferably in a healthcare or service environment required
* Patient Care/ Customer Service / Healthcare experience strongly preferred
What You Will Do
* Responsible for coordinating the clerical and administrative processes for entity care transitions management
* Responsible for managing deparmental mail box, phone lines and/or fax
* Supports transition planning activities as needed under the direction of care transition managers
* Calls vendors, physician offices, clinics, etc
* Faxes (electronic and traditional) documents as needed
* Makes copies or gathers other data as needed
* Collect and enter data as needed
* Completes other tasks and projects assigned
