CARE TEAM NURSE (LPN) Location: West Salem Clinic, 1233 Edgewater Street NW, Salem OR 97304 Job Status: Full-time, Monday-Friday Limited Duration: 6 months YOUR ROLE: As our Care Team Nurse, you will use a trauma-informed and whole-person approach to address patient wellness, acute illness, chronic disease, disability, and end-of-life needs in collaboration with their assigned primary care provider and care team. In this role, you will educate and motivate patients in areas related to self-care; dispense telephone advice to patients with minor acute problems; support the ongoing care management of patients with stable chronic disease; monitor patients during routine office visits; and perform of a variety of procedures that are within the scope of the professional nursing practice. QUALIFICATIONS: + Oregon Nursing License (LPN) + Has a current Basic Life Support for Healthcare Providers (BLS) training certificate consistent with AHA Guidelines for CPR/ECC, and covers breathing and cardiac emergencies - including CPR, AED, and obstructed airway - for adult, child, and infant patients.. + Clinic experience is preferred. + Ability to exercise independent judgment and decision-making. + Familiarity with vaccine admiration preferred.
