Job Description:

The Care Navigator provides member and operational support and transition duties, as assigned, for highest risk SelectHealth members by working collaboratively with the care management and utilization review team, patients, family caregivers, significant others, healthcare providers, payers, community-based providers, and other involved parties to support and promote effective, efficient, and patient-centered services.



Essential Functions

With support and oversight from a licensed Care Manager, Reviewer or Supervisor, works directly with Select Health members, their family, caregivers, and a multi-disciplinary care team in a collaborative process of outreach, screening, referrals, scheduling, planning, facilitation, navigation, coordination, and advocacy to ensure members safe and efficient navigation to the appropriate insurance, medical, behavioral health or community resource.Responsible for developing, documenting and implementing SMART goals, which will provide the appropriate resources to address social, physical, mental, emotional, spiritual, and supportive needs.  Advocates on behalf of members - collaborates, educates, communicates, and networks with healthcare providers, care team and community resources to ensure the member’s care planning needs are met and barriers addressed.Assists members in arranging follow up care and connecting members to community resources such as scheduling medical appointments, arranging DME or scheduling transportation.Supports member and care team to improve safe and effective transitions of careIdentifies members unable to adhere to a medical plan or members that require additional assistance and enrolls and follows these members in the applicable established model of care or care management program to re-enforce education, self-management, and other care planning actions needed. Follows policies and procedures to assess, establish and support a plan of care which incorporates the member's individualized needs and goals within the benefit plan throughout the care management process. Utilization management activities to support member care include support with acquiring clinicals, provider collaboration, EMR research, documentation of cases, length of stay support, high-dollar and actuary notification and facilitation of activities with physician and specialty reviewers, approval calls to members, assist with member communication.

Qualifications

Minimum Qualifications

Certification or associate degree, in a healthcare related field with demonstrated one year experience (e.g. LPN, MA, CNA, BSW, EMT or similar) OR 3 years Demonstrated experience and proficiency working in patient care, care management, transition / discharge planning, community social service, patient care coordination. Patient navigation.Experience with Advanced Microsoft Suite, including Teams (chat, whiteboard, task tracking) & Outlook. Capable of troubleshooting internet connection, basic computer hardware set-up, ability to customize computer settings and use multiple monitors.Strong written, verbal communication and conflict resolution skillsIntermediate computer skillsDemonstrated knowledge of health literacy and social determinants of healthStrong written, verbal communication skillsKnowledge of available health and community resources.Demonstrated experience in member and provider interactions

Preferred Qualifications

Post bachelor's degree in a clinical or related field.3-5 years' experience in a related clinical fieldAbility to work independently and be flexible in a rapidly changing environment.Experience working successfully in a remote environmentAbility to work independently, be self-motivated, have a positive attitude, and be flexible in a rapidly changing environment.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$22.64 - $34.48
