Job Summary and Responsibilities
Under direct supervision the incumbent identifies and evaluates potential clients as total person with focus on health limitations which impede independent living. The incumbent will certify level of care for the clients, provide medical information and training to care management staff, participate in California Community Transitions program and assist the development of organizational programs in response to changing health care services

Minimum qualifications
Three (3) years experience two (2) of which are in public healthAssociate of Science degree in nursingValid current California license to practice nursingPreferred Qualifications
