Role Description At Assisting Hands Brevard, we’re seeking a Care Manager who not only thrives in people-focused management but also understands the power of marketing in building strong relationships. The ideal candidate is a proactive, self-sufficient multi-tasker with a natural sense of empathy, combined with exceptional organizational skills. This full-time position is ideal for an experienced clinician (such as an LPN or Social Worker) who is passionate about both care coordination and business growth. In this role, you’ll be a part of our care management team to oversee and enhance our non-skilled home care services, while also playing a key part in expanding our agency’s reach within the community.
Qualifications Clinical experience in a healthcare setting Elder Care and Social Services experience Case Management and training skills Strong communication and interpersonal skills Ability to prioritize and multitask in a fast-paced environment Knowledge of local resources and community services
Primary Duties Include: Meets with new clients to hear their needs and explain how our services can bring benefit to them. Complete intake paperwork and provide orientation to services with Assisting Hands® Home Care. Provides on-site training as needed with counseling, coaching and educating non-skilled Caregivers and Clients. Completes new Plan of Care, as needed, and communicates accordingly with the Caregivers and Scheduling Coordinators. Conducts telephone follow-up of Prospective Clients: answers questions, explains our services, strives to make a connection with the Prospect/Prospect’s family, offers to schedule In-Home Assessment/Intake. Identifies marketing opportunities associated with current clients, such as home health agencies, doctor offices, guardian companies, case managers, etc. Maintains appropriate and daily documentation in company's web-based software, WellSky Personal Care (formerly known as ClearCare).
Hours: Full-time, 5 days per week. Hours are flexible with approval from management.

Compensation: $62,500.00 - $65,000.00 per year
