POSITION SUMMARY :

+ The Care Manager is a key member of the CTI component, responsible for delivering intensive case management services to individuals with complex needs, including those transitioning from institutional care. The Care Manager will work collaboratively with discharge teams, community providers, and internal staff to ensure seamless service delivery. This role is crucial in coordinating housing, mental health services, and developmental disability services, while maintaining continuous care monitoring and adapting plans as needed to reflect the changing needs of individuals.

+ POSITION DUTIES :
+
+ Provide intensive case management services, ensuring that individuals have access to necessary resources and support for community integration.
+ Coordinate housing, link individuals to mental health and I/DD services and manage ongoing care needs.
+ Collaborate with hospital discharge teams, community providers, and CPU clinical staff to facilitate seamless transitions.
+ Monitor client progress, adjusting care plans to reflect evolving needs, and ensure that care coordination aligns with individual recovery goals.
+ Advocate for clients' needs, empowering them to navigate systems and access services that sup port long-term stability and recovery.
+ REQUIREMENTS :
+ Bachelor's degree in social work, psychology, or related field required.
+ Minimum of two (2) years of experience in care management, particularly with mental health and I/DD populations.
+ Strong organizational and problem-solving abilities, with a capacity to manage complex cases and coordinate across systems
+ Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures . Adheres to the agency's corporate compliance program, including the standards of conduct, the NYS and federal false claims act, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
