Care Management Social Work Supervisor - Pop Health Case Management - Day
**Schedule** Full Time, Day Shift (80 hours biweekly) Hours: Monday - Friday, 8:00am - 4:30pm **General Summary** Supervises and oversees the work activities of Care Management social work and social services staff and works in a team leadership model to develop and maintain departmental operations that are consistent health system, ambulatory, inpatient, and community needs. May carry a caseload and provides services to patients and families including care coordination across the continuum, transitions of care support, and referral to community services. **Essential Functions:** + Assists in selecting staff, trains/orients personnel, evaluates performance, prepares work schedules and makes recommendations for personnel actions. + Assists in developing budgets. + Identifies needs in program planning and facilitates program development. Supervises patient care, family communication, and record keeping. + Collaborates with inpatient, ambulatory, population health, and community programs, as well as external agencies and payer partners, to support coordinated patient care across the continuum. + Provides direct service to patients including psychosocial assessment, interviewing patients and families, identifying barriers to care, care planning, and coordination of appropriate clinical and community resources. + Prepares and maintains written documentation as required by the profession and the department. + Provides psychosocial support, crisis intervention, and resource coordination appropriate to the care management setting across inpatient or ambulatory environments. + Oversees the implementation and daily operations of the lean daily management system. + Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. + Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops. + Acts as liaison with inpatient teams, ambulatory practices, interdisciplinary clinical partners, and community agencies to facilitate problem solving, care coordination, and service integration. + Oversees/Conducts CM SW staffing and assignments, performance evaluations and evaluation and testing of competencies (as applicable). Provides orientation, training and continuing education of staff and initiates or makes recommendations for personnel actions. **Common Expectations:** + Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s). + Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause. + Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. **Qualifications** **Minimum Education:** + Bachelors Degree Degree in Social Work preferred. Other related field accepted. Required + Masters Degree Degree in Social Work. Preferred **Work Experience:** + 2 years Relevant experience. Required + 1 year Supervisory experience.
