Winter Park Memorial Hospital has continuously served the residents of Winter Park and its surrounding communities for more than 50 years
Chartered in 1951, the hospital has grown from the visionary efforts of a handful of Winter Park residents and community leaders, to a 307-bed acute care facility that is a model of community health and wellness
Over the years the hospital has continually expanded to meet the needs of the community, adding an upscale obstetrics and Level II Neonatal Intensive Care Unit at The Dr. P. Phillips Baby Place, cancer care at the AdventHealth Cancer Institute, Winter Park, and state-of-the-art surgery, recovery and rehabilitation at the AdventHealth Orthopedic Institute
The role you'll contribute: The Care Management Assistant (CMA) is responsible for implementing patient discharge plans as directed, assisting with coordination of clinical data collection, and ensuring third-party payer notifications and authorizations are secured and documented as a support function for Care Management and Care Coordination staff. Interacts with representatives from insurance companies, managed care organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies as required. Responsible for assisting in coordinating care and services for assigned patients under the direction of the RN Care Manager and/or Social Worker and in collaboration with the patient/family. The CMA establishes a general working knowledge of payer-specific documentation and authorization requirements, and regulatory requirements regarding the clinical review process, provision of services, and post discharge services. Ensures discharge arrangements are in compliance with payer requirements, and regulatory guidelines. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Implements patient discharge plans prepared by RN Care Manager and/or Social Worker team members.
Assists Care Managers by performing a variety of tasks as required. These may include but are not limited to:
Checking for nursing home, rehab, adult foster care and other facility vacancies/placement
Photocopying and faxing appropriate information to above facilities to help facilitate placement as requested by RN Care Managers and/or Social Workers
Makes referrals for community agencies
Makes home care referrals, ensuring forms are completed correctly and compliantly
Explores availability and updating of community resources
Assists patients and/or their families/caregivers with concrete needs such as transportation, emergency medications, medical equipment, etc.
Assists patients post discharge who have questions related to discharge plan implementation
Contacts third-party payers to obtain and/or confirm authorization for services and communicates necessary related clinical information, when required.
Assists Care Manager/Care Coordination department in collecting data related to patient-care outcomes and auditing data for accuracy.
