Job Details Responsibilities: Assist Care Coordination team with getting authorization (i.e.) Transportation, SAR, Homecare, etc. Assist Care Coordination team with setting up transportation Assist with making packets for SNF and others as needed Assists with maintaining Departmental files, reference information, shared drive Assists with the ordering of supplies and equipment. Maintains and distributes communications within Care Management office Contacts insurance companies for follow up regarding authorization for services Submits clinical on insurance portals to facilitate authorizations. Contacts SNFs to provide details on returns or new admissions Sends out dialysis packets for resumption and new placements Assist and complete the appeal process for LIVANTA and IPRA Qualifications: High School Diploma or equivalent Previous relevant healthcare experience preferred. Excellent computer skills, attention to details, interpersonal and organizational skills required. Previous relevant healthcare experience preferred Salary Range USD $25.44 - USD $25.44 /Yr.
