demonstrated expertise in the assessment and treatment of patients along the continuum of care Identifies risk factors for both psychosocial interventions and discharge needs key member of the interdisciplinary health care team identifies discharge barriers develops and implements safe discharge plans regardless of complexity of patientrsquos discharge needs in a timely manner Ability to address all aspects of patientrsquos psychosocial needs including leading family meetings providing bereavement support and grief counseling advocacy with community agencies and within the acute care
