Job Duties:
· Work onsite at HH Urgent Care in Madison on designated days to facilitate referrals, coordinate patient services, and utilize the EMR system effectively.
· Collaborate directly with HH Urgent Care physicians and staff to ensure timely processing of referrals and patient information.
· Serve as the primary liaison for referral management at HH Urgent Care, ensuring patients are connected to appropriate specialists, imaging, and procedures within the HH Health System.
· Provide onsite support for referral authorizations and scheduling, ensuring accurate and efficient documentation of patient needs.
· Maintain up-to-date knowledge of EMR processes at HH Urgent Care and troubleshoot referral or scheduling issues as needed.
· Work with HH Urgent Care teams to identify and resolve gaps in referral processes to enhance patient satisfaction and continuity of care.
· Collaborate with the Communication Center and Marketing's New Movers Program to identify and assist community members in connecting with Huntsville Hospital services.
· Coordinate with HH Urgent Care physicians and staff to ensure referrals from all North Alabama locations are facilitated and directed to Huntsville Hospital-owned facilities and providers in a timely and efficient manner.
· Coordinate authorizations for health care services within the health system to assure high quality continuity of care for patients, employees, and clients with a high level of customer service.
· Work with HH Health Specialists offices including the HH Heart Center and ACO in placement of patients in need of a primary care physician.
· Support the Transition of Care Case Management team in securing providers for inpatients before discharge to facilitate timely home health and DME services.
· Assist Occupational Health Group clients by coordinating comprehensive annual physicals for expatriates, including special visits, imaging, procedures, and translation services as needed.
· Represent Huntsville Hospital at health events by maintaining an organized, engaging promotional booth and serving as a key point of contact for client employees, providing information and guidance on navigating the health system.
· Educate client employees on health insurance benefits, including co-pays, deductibles, provider networks, pre-authorizations, and pre-certifications.
· Assist in keeping the referral database up to date and distribute to all relevant stakeholders.
· Document and follow up on patient experiences in a timely manner.
· Track patient interactions within the health system and update customer relationship management (CRM) records to ensure high-quality continuity of care.
· Assist with securing physician referrals required for specialists, imaging and certain procedures, along with updating HH Health hospital facility rosters. These include recognition of physician practices accepting new patients and system database updates monthly of adds or deletes in employed and staff physicians.
· Foster positive relationships with employers, employees, physicians, and medical staff.
· Communicate client concerns and resolve issues promptly, keeping all relevant stakeholders informed.
· Collaborate with the Corporate Care Business Development Team to promote and coordinate occupational, preventive, wellness, and general healthcare services.
· Provide concierge services to assist client employees and their families in finding providers, scheduling appointments, lab work, and imaging, ensuring seamless internal referrals.
