Various (United States of America)Job Summary:\rThe Cardiovascular Disease Coordinators will serve as a resource for patients, physicians, and staff, performing in the\rrole of education, quality improvement champion and consultant for patients with cardiovascular disease. This position\ris able to function independently and demonstrate self-motivation. Reinforce adherence with best practice guidelines.\rWork closely with the AU Health faculty and management team to analyze data and formulate Quality Improvement\rprojects based on findings.\rCore Responsibilities and Essential Functions:\rClinical - Demonstrates understanding of CV Disease and the care of this patient population. Ensures a process\rof identifying these patients in the facility. Responsible for organizing services and systems care of these\rpatients. Works collaboratively with clinical staff to ensure evidenced-based practice guidelines are utilized\rto optimize patient care. Actively participates in the development and implementation of standardized care.\rPerformance Improvement - Develops a continuous process of monitoring, assessment, and management\rthat is supported by the registry database. Coordinates daily data processing and monitoring the\reffectiveness of established evidenced-based practices.\rEducation - Is seen as a clinical expert in the specialty by obtaining certification in the specialty. Provides staff\reducation as needed. Able to collaborate and work across external organizations and internal\rmultidisciplinary departments. Develops a program to engage referring facilities to improve outcomes for\rpatients. Assists with moving patients through the complex healthcare transition as necessary.\rRegistry and Accreditation - Gathers key data elements, assuring the integrity of the data, consistently\rutilizing national standardized definitions and uses the data to improve the care of the cardiovascular patient.\rCommunicates effectively with the primary care team, including physicians and staff to identify and resolve\rvariances in care. Evaluates outcomes against benchmarking data.\rCommunity Outreach -Engages patients and the community by developing educational materials/offerings\ron cardiovascular disease. Develops a process for training of external organizations to improve the transition\rof care.\rPerforms other duties as assigned\rComplies with all Wellstar Health System policies, standards of work, and code of conduct.\rRequired Minimum Education:Bachelors Nursing\r\rRequired Minimum License(s) and Certification(s):\rAll certifications are required upon hire unless otherwise stated.BLS-I - Basic Life Support - Instructor or BLS - Basic Life SupportRN-COMPACT - RN - Multi-state Compact or RN - Reg Nurse (Single State)\rAdditional License(s) and Certification(s):\rRequired Minimum Experience:\rMinimum 3 years Cardiovascular acute care nursing Required or\rMinimum 5 years Cardiovascular acute care nursing Preferred and\rRequired Minimum Skills:\rProficient with Microsoft office: Powerpoint, Excel, Access, and Word HighJoin us and discover the support to do more meaningful work—and enjoy a more rewarding life.
