You'll provide essential clinical oversight and guidance for our cardiac programs, from coaching providers to reviewing and advising on educational pathways. Your expertise will directly influence our strategic direction, particularly in the ever-evolving landscape of value-based care. Position Highlights Clinical Leadership: Provide coaching and support to cardiac providers, ensuring the implementation of clinical pathways and protocols that align with the latest ACC/AHA guidelines and CMS quality metrics. Strategic & Operational Oversight: Collaborate with hospitals and payers to shape our integrated post-acute care strategy. You'll participate in C-Suite meetings and high-level payer negotiations, offering your expertise and bringing innovative ideas for new medical models. Quality Improvement: Analyze outcome reports to identify service gaps and develop strategies that improve care delivery, operational efficiencies, and resource utilization. You will be instrumental in optimizing performance under Value-Based Purchasing (VBP) programs and Five-Star Ratings. Education & Documentation: Serve as a subject matter expert, educating physicians and staff on documentation best practices to support medical necessity and maximize reimbursement. You will monitor and advise on utilization risks, ensuring our services are both effective and compliant. Stakeholder Engagement: Actively engage with a wide range of partners—from payer partners and hospital discharge planners to regulatory bodies—to position our programs as the premier high-acuity post-acute care solution.
