Cardiac Surgery Program Coordinator - Full time
**_Full-Time - 1.0 FTE/80 Hours Biweekly_** **_Day Shift - 7:30am-4pm_** **_No weekends or holidays_** **General Summary** Plans, organizes and directs and performs a wide variety of administrative and fiscal functions for the Cardiac Surgery Program. Partners with the medical, surgical and clinical staff of multiple departments, to coordinate all aspects of treatment for the Cardiac Surgery patient including, support services, patient care/services from the inpatient setting to an outpatient setting in order to improve patient outcomes. Facilitates the appropriate use of medical resources in the inpatient setting using established standards as guidelines. Functions as the central figure in the Cardiac Surgery Program. Collaborates with the health care team and the patient /family to accomplish agreed upon outcomes. **Duties and Responsibilities** + Introduces patient to the Wellspan Cardiac Surgery Program and the Cardiac Surgery facilities and unit(s). + Serves as a liaison between the patients, families and healthcare team throughout the process of diagnosis and treatment. + Coordinates with the interdisciplinary team to develop a plan of care. + Links patient/family to appropriate organizational or community resources and facilitates access. + Serves as a consistent contact for patient/family throughout the process of diagnosis and treatment. + Provides individualized support to patient/family, during pre-op and inpatient period, as well as after discharge. + Works to coordinate appointments for consultations and provider visits. + Reinforces and assesses pre- and post-operative instructions and provides further education as appropriate. + Navigates and supervises program development and/or expansion of the Cardiac Surgery Program. + Communicates and collaborates regularly with physicians and health care personnel involved in the patient's care across the continuum of care, in order ensure continuity of care and implementation of appropriate care models. + Collaborates with leadership and the Corporate Quality Team to provide program evaluation, process improvement, tracking metrics and patient outcomes, to optimize the patient care experience. + Leads monthly, multi-disciplinary Quality meetings and helps develop and monitor PI plans to ensure delivery of quality services for patients. + Works with Cardiovascular Leadership to develop monthly reports as requested on volume based on type of procedure, referring physician, hospital transfers etc. + Maintains a thorough knowledge and understanding of evidence based/best practice. + Revises and monitors educational materials to ensure that they are coordinated, understandable and meet the program standards. + Protects patient confidentiality by promoting professional staff communications. + Works with case management to make arrangements for discharge needs (SNF, Cardiac Rehab, Home Health) in collaboration with patients, families and care team as needed. + Collaborates with physicians, other health care provider, and social work to ensure continuity of care and implementation of appropriate care models **Common Expectations:** + Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required. + Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. + Acts as a resource to other healthcare professionals in the practice setting on disease and case management topics and operational approaches + Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards. + Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation. **Qualifications** **Minimum Education:** + Bachelors Degree Required **Work Experience:** + 3 years Nursing experience Required + Cardiology nursing experience Preferred **Licenses:** + Licensed Registered Nurse Upon Hire Required or + Registered Nurse Multi State License Upon Hire Required **Courses and Training:** + Certification in appropriate nursing specialty.
