Responsibilities and Essential Job Functions
Assists in completion of transplant registry data reporting requirements for all post-transplant patients.Attends weekly BMT patient review meeting and maintains patient update log; updates log throughout the BMT Case Management Log, a summary of status of all active patients.Coordinates gathering of PHI, and collates and files accordingly within BMT department and hospital medical records patient charts.Demonstrates competence in organizing and prioritizing disease specific information.Demonstrates knowledge of disease types specific to BMT population, including diagnosis, history, treatment modalities, and ongoing monitoring.Functions as the coordinator for emergency transfer and admission of acute leukemia patients.Functions as the intake coordinator for all post-transplant follow-up patients.Prepares post-transplant patient specific timeline summary.Schedules all appropriate labs and test procedures for all BMT patients according to the requisite BMT post-transplant guidelines.Pre-Transplant Responsibilities: Attends weekly BMT patient review meeting and maintains patient update log; updates log throughout week.Coordinates gathering of PHI, including CDs and slides, and assembles new patient chart.Demonstrates competence in organizing and prioritizing disease specific information.Demonstrates knowledge of disease types specific to BMT populations, including diagnosis, history, treatment modalities, and ongoing monitoring.Functions as the coordinator for emergency transfer and admission of acute leukemia patients.Functions as the intake coordinator, the first department contact person for outside referrals.Prepares new patient specific timeline summary and sends out new patient notification to BMT team.Updates the BMT Case Management Log, a summary of status of all active patients.Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department.These statements are intended to describe the essential functions of the job and are not intended to be an exhaustive list of all responsibilities.
