member of Community Health and Engagement a registered nurse responsible for the provision of an organized program directed toward community education and health improvement Utilizes nursing skills to analyze develop and implement community education programs based on departmental prioritiesAssists with colleague education related to caring for people from different cultures and diversity issuesWorks in conjunction with other members of Community Health and EngagementJob responsibilitiesmiddot Aggregates data to support community education and preventative health programsmiddot Respond to public requests for health education programsWorks with physicians clinical staff and community partners to staff events Supports interventions targeted toward population health needs and program enhancementsmiddot Works with Human Resources and others on diversity issues related to colleagues patients and communitymiddot Coordinates and oversees various screening programs throughout the Hartford Regionmiddot Provides consultation and education to selected patient populations to facilitate health promotionmiddot Serves as hospital liaison to community organizations related to health education and community needsmiddot Maintains awareness of resources and regulations at the local state and federal levelmiddot Works closely with the Regional leadership team to ensure that the Hartford Region provides healthcare and community education in concert with health needs assessmentmiddot Demonstrates knowledge of care for required age specific populations necessary to carry out safe and effective performance of dutiesmiddot Participate in hospital functions and events that support and health and engagementMay be required to vary shifts to meet the needs of patients and
