As a Community Health Worker (CHW), you will play a vital and unique role in linking diverse and underserved populations to health and social service systems. You will primarily work alongside medical providers, primary care teams, and other agencies in the community with specific target populations to improve patient care and outcomes. In this role, you will have the exciting opportunity to work with patients/families from pregnancy through the first year of the baby’s life helping to address infant and maternal health disparities.
This trusting relationship enables you to serve as a liaison to facilitate access to services and enhance the quality and cultural competence of service delivery. You will also help increase health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy.
A caregiver in this position works days, 8:30 a.m. -- 5:00 p.m.
A caregiver who excels in this role will:
Establish trusting relationships with patients and their families while providing support, encouragement and feedback. Coach patients in effective management of their chronic health conditions and self-care. Convey the purposes and services of a program to the user population and the impact the program or service would have. Assist patients in understanding care plans and instructions. Visit patients in their homes as needed to assess the patient, their living conditions, and meet with family members or caretakers. Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place. Advocate for patients and act as a liaison between the patient/family and community service agencies (schools, hospitals, support groups, etc.). Help patients in utilizing resources and completing eligible applications. Assist patients in accessing health related services, including obtaining a medical home, appropriately using the medical home, overcoming barriers to obtaining needed medical care and social services. Facilitate communication and coordinate services between providers. Motivate patients to be active, engaged participants in their health. Effectively work with people (staff, clients, doctors, agencies, etc.) from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions. Build and maintain positive working relationships with the clients, providers, nurse case managers, agency representatives, supervisors and office staff.Minimum qualifications for the ideal future caregiver include:
High School Diploma/GED Valid Driver’s License (VLD) Successfully complete the Community Health Worker (CHW) certification within 6 months of hire date and be certified as a CHW through the Board of NursingKnowledge of community resources Ability to develop trusting and effective working relationships Strong information gathering, interviewing and time management skillsPreferred qualifications for the ideal future caregiver include:
Two years of community service experienceMedical Assistant training or Medical Assistant Certification Medical terminology and one year work experience in a medical office settingKnowledge of the Electronic Medical Record (EMR) Written and verbal communication in English and SpanishEPIC experiencePassionate about helping people in the communityOur caregivers continue to create the best outcomes for our patients across each of our facilities.
