This role involves conducting client assessments, maintaining accurate documentation, and supporting eligibility determinations for nutrition services. You will also contribute to the success of the Meals on Wheels program and work collaboratively with a mission-driven team. Responsibilities:· Conduct home and telephone visits to assess clients’ ability to receive and store meals· Perform accurate client assessments and reassessments during visits· Maintain and organize electronic documentation of client files· Determine eligibility of potential clients for nutrition services· Provide nutritional information and referrals while collaborating with the Client Services Team Qualifications and Requirements:· Bachelor’s Degree in Social Work preferred· Bilingual in Spanish is desirable· One year of client casework experience preferred· Passion for serving senior citizens and working with low-income elderly populations· Must maintain timely and accurate computerized and paper records· Reliable transportation required; must drive to client sites daily· Physical demands include walking on uneven surfaces, bending, sitting while driving, and working in varying weather conditions and home environments Benefits and Perks:· Pay Rate: $17 to $18/hour· Medical, dental, and vision benefits for employees and family· Holidays· Training & Growth opportunities Your New Organization:The organization serves the greater Houston area through four key service areas: Meals on Wheels, Refugee Services, Interfaith Relations and Community Partnerships, and Volunteerism and Civic Service.
