Behavioral Transfer Care Coordinator

Job Summary and Responsibilities

As our Transfer Center Coordinator, you will help patients and their families navigate the complex transfer process so they can receive timely and appropriate care.

Every day you will coordinate patient transfers between facilities and external healthcare providers. You will be expected to assess patient needs, communicate effectively with healthcare teams, and ensure a smooth transition of care.

To be successful in this role, you must possess strong clinical judgment, critical thinking skills, and excellent communication abilities.

Provide the physician and the health care team with recommendations and information which will assist in developing an overall plan for the patient’s care upon consultation.Partner with providers and hospital staff in effective discharge planning.Educates patients and families by providing facts about diagnosis, treatment plans and available community resources.Collaborates with the physician(s) and health care team and serves as a patient advocate and liaison with community agencies.Develops and maintains good working relationships with community treatment network to ensure the availability of quality services.Coordinates care with other health care community providers. Connects patients and families with these providers/services.
