Summary
Manages a work queue set up specifically for referrals for behavioral health services. Completes clinical intakes to determine the services which best fit the patient's needs and works with the patient, referring practice, and/or community behavioral health providers to facilitate a connection for services within the healthcare continuum.

Job DutiesManages a work queue of referrals for all patients seeking behavioral health services. Provides comprehensive documentation that is presented in a clear and concise manner and completes a comprehensive chart review to determine the appropriateness of referral. Schedules the patient, provides external resources, or routes the chart to the program manager for further review depending on the needs of the patient. Directs the patient to a higher level of care other than outpatient if the patient requires emergency services (ED, crisis, or police involvement) after consulting with leadership. Provides support and assistance to medical practices on the process of referring patients for behavioral health services. Completes all documentation in a timely manner and within best practice standards.
Minimum QualificationsBachelor’s Degree in Social Work, Psychology, or Social Services related field. 1 year working in mental health, substance abuse, or a social service agency.
Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
