The Behavioral Health Manager coordinates care delivery and conducts reviews of services to determine the appropriate level of care utilizing standardized testing measures and safe, clinically appropriate care. Provides pre-admission and concurrent reviews for Behavioral Health levels of care and coordinates daily administrative and clinical operations.   Essential Duties & Responsibilities: Works to ensure linkage and integration of social work and behavioral health principles into the medical care provided to each resident. Provide timely and accurate completion of resident clinical assessments (i.e., cognitive, depression, anxiety, etc.) and other forms as required in accordance with federal, state, and local regulations and facility/company policies and procedures. Provide individual and group counseling services as needed for those identified through the assessment process outlined above. Work closely with the facility external mental health providers to insure all resident psychosocial/psychiatric needs are identified and services are provided. Provide direct consultation to facility staff with care management and regulatory standards in Behavioral Health. Participate in care planning by supporting emotional and psychological needs of the residents. Conduct utilization reviews, providing and summarizing clinical information to supervisors and physicians to ensure appropriate medical care coordination and service delivery. Provide continual education to facility staff around Behavioral Health through hands on demonstration during normal care delivery routines. Participate in Referral Management of all potential Admissions with Behavioral Health concerns including but not limited to on-site reviews. Serve as an information source for staff, residents, and families. Identify opportunities to enhance access, specialty care services, and reduce service gaps within the behavioral health provider network (i.e., Home and Community Based Services, Alzheimer’s Association, etc...). Maintain testing and charts review notes for each resident as required by company policy and state/federal regulations. May assist with management duties including, but not limited to, hiring, training and developing, coaching and counseling, and terminating department staff, as deemed necessary. Maintain confidentiality of resident and facility records/information. Other special projects and duties, as assigned.     Job Requirements: Certificate or licensure as RN with training in dementia and/or psychiatric care, Clinical Nurse Specialist, MSW, LMHC, or Licensed Psychologist preferred. Two (2) to four (4) years experience in mental health and/or long-term care. Must have a current/active CPR certification. Detail oriented and excellent analytical skills. Ability to work both independently and as part of a team. Strong oral and written communication skills; ability to interact within all levels of the organization. Highest level of professionalism with the ability to maintain confidentiality. Effective skills utilizing Microsoft Office products. Database experience preferred. Demonstrated ability to successfully plan, organize, implement, and manage projects within a health care setting. Excellent assessment, listening and counseling skills, Genuine caring for and interest in elderly and disabled people in a nursing facility. Relates to and works with the ill, disabled, elderly, emotionally upset, and at times hostile people in the facility. Customer service oriented with the ability to work well under pressure. Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity. Ability to work with minimal supervision, take initiative and make independent decisions. Ability to deal with new tasks without the benefit of written procedures. Approachable, flexible, and adaptable to change.     Environment: Work will be performed throughout all areas, including in resident rooms, and on carpeted and/or tiled floors. Work will be performed there routinely around other co-workers, healthcare staff, residents, and guests. Due to the nature of facility’s business, worker may be exposed to occasional slippery floors, object on floors, chemicals, sharp objects, hazardous materials, and waste (including human), blood­ borne pathogens, and communicable diseases, as well as high-stress medical and/or life-threatening situations. Physical and Sensory Requirements: Moderate physical activity: Push, pull, move, and/or lift a minimum of twenty (20) pounds to a minimum height of three (3) feet and be able to push, pull, move, and/or carry such weight a minimum distance of fifty (50) feet. Standing and/or walking for more than four (4) hours per day. Bending and/or stooping for more than one (1) hour at a time.
