Depression, anxiety and other mental health concerns can hinder the life patients want for themselves. At Cleveland Clinic, caregivers understand these issues are just as real as those that affect the body. Cleveland Clinic offers comprehensive mental health programs, allowing patients to receive the personalized, confidential and judgment-free support they need. As a Behavioral Health Social Worker on our team, you will utilize your specialized clinical knowledge and advanced clinical skills in psychotherapy, clinical counseling, diagnostic interviews, assessments and treatment plans to care for patients of all ages, backgrounds and needs. Your work will directly impact their lives as you work to help them improve their overall well-being.
A caregiver in this role works days from 8:00 a.m. – 4:30 p.m.
A caregiver who excels in this role will:
Coordinate the treatment and discharge planning needs for psychiatrically acute patients.
Conduct a detailed psychosocial assessment.
Offer crisis intervention to patients and families with complex psychosocial needs.
Act as Liaison with community agencies to coordinate a safety plan that represents the recommendations of the interdisciplinary team.
Participate in the development and implementation of an appropriate assessment based interdisciplinary treatment plan.
Complete comprehensive psychosocial assessment.
Participate in interdisciplinary rounds.
Communicate with interdisciplinary team regarding the discharge planning status of all patients assigned to them.
Supervise and/or train social work graduate students and interns.
Intervene with families exhibiting complex behaviors and dynamics.
Communicate with and advocate for patient and family members.
Intervene in cases involving abuse and neglect.
Coordinate legal resources to ensure protective services and guardianship as necessary.
Provide consultation to medical floors, ER and outpatient areas as needed for patients with complex psychosocial needs.
Meet core social work competencies.
Minimum qualifications for the ideal future caregiver include:
Master’s Degree in Social Work, Counseling or a related field
Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) or Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) with no record of substantiated ethics or patient abuse reports
Critical thinking skills, decisive judgment and the ability to work unsupervised or with minimal supervision
Preferred qualifications for the ideal future caregiver include:
Behavioral health experience
Physical Requirements:
A high degree of manual dexterity to produce materials on PC; normal or corrected vision, extensive sitting and frequent walking, occasional lifting or carrying up to 25 pounds.
Personal Protective Equipment:
Follows Standard Precautions using personal protective equipment as required for procedures.
