Overview

A Bachelor's Social Worker is responsible for collaborating with internal and external team members to identify barriers to care and resource needs for patients and families. The Bachelor's Social Worker supports patients, families and teams by conducting assessments; providing emotional support; identifying resource needs, making community referrals, and advocating for families. Bachelor's Social Worker collaborates with colleagues to enhance and meet strategic goals.

Responsibilities Psychosocial AssessmentComplete assessments to evaluate patient and family psychosocial risk factors through evaluation of prior functioning levels, availability of support systems, past and current coping skills, with the focus on identifying barriers and risk factors to create treatment plans that maximize health outcomesCare Coordination and Patient/Family SupportDeveop an effective working relationship with patients and families to engage, collaborate, educate, advocate and enhance coping skills in order to improve functioning with respect to self-determinationPractice Management Maintain timely and accurate documentation of all interactions in the electronic medical record in accordance with department policy Qualifications Bachelor's Degree Social WorkBSW from Accredited Program and1-2 years experienceStudent practicum in hospital social work would count as 1 year experienceLBSW MO Licensed Bachelor's Social Worker, must have a MO or KS license upon hire and then be dually licensed in both states within 90 days of employment, required upon hireLBSW KS Licensed Bachelor's Social Worker, must have a KS or MO license upon hire and then be dually licensed in both states within 90 day of employment, required upon hire Benefits at Children's Mercy

Our pay ranges are market competitive. The pay range for this job begins at $24.41/hr, but your offer will be determined based on your education and experience.
