Assessment Referral Coord
**Job Summary** The Assessment Referral Coordinator supports patient care by coordinating referrals, authorizations, and follow-up appointments in collaboration with physicians and other care team members. This role ensures timely scheduling, accurate documentation, and clear communication with patients to facilitate continued access to services and high-quality care. **Essential Functions** + Coordinates patient referrals, authorizations, and appointments for specialty care, diagnostics, and follow-up services in accordance with provider orders. + Communicates with patients and families to explain next steps, provide instructions, and ensure understanding of referral and appointment requirements. + Obtains prior authorizations and referral approvals as needed, and accurately documents information in the electronic health record. + Schedules appointments and follows up with external providers to confirm referral completion and collect consult notes. + Responds to incoming calls and referral requests, triaging appropriately and ensuring timely processing. + Maintains accurate and organized patient records, ensuring documentation is complete and submitted according to policy. + Performs other duties as assigned. + Complies with all policies and standards. **Qualifications** + Associate Degree in Healthcare Administration or related field preferred + 0-2 years of experience in a healthcare administrative or referral coordination role required **Knowledge, Skills and Abilities** + Knowledge of referral processes, insurance requirements, and prior authorization procedures. + Proficiency with electronic health record systems and scheduling software. + Excellent verbal and written communication skills. + Strong attention to detail and organizational skills. + Ability to manage multiple tasks in a fast-paced environment.
