Ambulatory Clinical Pharmacist- UVA Specialty Pharmacy
The UVA Specialty Pharmacy centralized clinical pharmacist proactively works with patients with chronic and/or rare disease states to provide specialty pharmaceuticals and therapy management services. Through our clinical management program, this individual promotes enhanced patient understanding and increased adherence to medication regimens, prevents and detects adverse drug events, as well as patterns of over-use and under-use of specialty medications. Utilizing a combination of patient education, provider communication and medication access services, the UVA Specialty Pharmacy’s clinical management program seeks to assist patients in obtaining the best possible outcomes for their specialty medication therapy. This individual will promote UVA Specialty Pharmacy’s culture of caring and communication, which fosters patient empowerment through education and support.


Services include:
• Chronic Disease Management:
Reviews patient-specific data (labs, diagnostics, vitals, and adherence trends), conducts telephonic or virtual clinical assessments, monitors for drug interactions, contraindications, and therapeutic duplications, and provides timely pharmacotherapy consults to providers. Tracks clinical goals and intervention outcomes.
• Medication Counseling & Education:
Educates patients on proper use of specialty medications, including complex administration techniques, timelines for efficacy, side effect monitoring, and supportive lifestyle measures. Offers guidance on storage, disposal, travel considerations, and other patient-specific concerns.
• Prescribing Support:
Supports providers in selecting evidence-based, cost-effective therapies and serves as a clinical drug information resource for specialty treatment areas.
• Access and Benefit Support:
Partners with UVA Specialty Pharmacy’s medication access team by providing clinical information and documentation to support prior authorizations and benefit investigations.
• Process Improvement and Clinical Advancement:
Participates in continuous quality improvement initiatives, supports cross-functional collaboration, and contributes to workflow enhancements to advance specialty pharmacy practice.

Provides clinical care to patients.

Supports and enhances the quality of care in ambulatory practice areas.

Enhances patient access to affordable care.

Supports the educational and professional development of healthcare professionals.

Engages in professional self-development.

In addition to the above job responsibilities, other duties may be assigned.

Position Compensation Range: $52.46 - $83.94 Hourly

MINIMUM REQUIREMENTS

Education: Bachelor’s in Pharmacy with experience or Pharm.D. from an ACPE accredited College of Pharmacy. Degree required.  
Experience: PGY-1 residency required or equivalent clinical experience.
License/Certification: Commonwealth of Virginia State Board of Pharmacy License required. American Heart Association (AHA) Health Care Provider BLS certification required. Board Certification or equivalent certification specific to clinical area highly encouraged. 
 

PHYSICAL DEMANDS

Job requires sitting for prolonged periods; Repetitive motion: typing.  Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs.  May be exposed to fumes, chemicals, blood/body fluids and infectious disease.
