AIM Palliative RN
**Overview** We are seeking a compassionate and experienced **Registered Nurse (RN)** to join our **Advanced Illness Management (AIM) Palliative Care Team** . This is a field-based position that provides in-person and telephonic care coordination, education, and assessments to patients with serious or life-limiting illnesses. The **AIM Palliative RN** will collaborate with an interdisciplinary team including Nurse Practitioners, Social Workers, and Primary Care Providers.
