We’re seeking an Afterhours Clinical Support Registered Nurse (RN) to join our dedicated hospice team. In this vital role, you will provide supportive care to patients and families after normal business hours, ensuring that comfort, dignity, and compassion remain constant—day or night.
Reporting to the Manager, Afterhours Clinical Support, the RN will manage and direct an interdisciplinary group consisting of Registered Nurses, Licensed Practical/Vocational Nurses, Hospice Aides, and support staff to enhance the quality and continuity of hospice care.
Essential Functions of Position
Provide and document the highest quality of care in accordance with the Plan of Care.
Function as the Administrator on-call during afterhours and weekend shifts.
Perform initial and ongoing patient assessments as required.
Develop and implement individualized Hospice Plans of Care in collaboration with physicians and interdisciplinary team members.
Maintain accurate and updated medical records, including all assessments, notes, and care plans.
Monitor visit scheduling, admissions workflow, and documentation compliance.
Communicate changes in patient condition to the attending and hospice physicians.
Collaborate closely with the hospice interdisciplinary team to provide physical, psychosocial, emotional, and spiritual support to patients and families.
Participate in the Quality Assessment and Performance Improvement (QAPI) program.
Maintain a high level of knowledge in symptom management and end-of-life care best practices.

About You
Education
Registered Nurse with a minimum of an Associate’s Degree in Nursing required.
Bachelor’s Degree in Nursing (BSN) preferred.
Experience
Minimum of one (1) year of hospice nursing experience required.
At least one (1) year of management or team leadership experience preferred.
Licenses, Certifications, and Requirements
Current RN license in the state of employment.
Reliable transportation and ability to travel as needed.
Proficiency with EMR systems and remote communication tools preferred.
Knowledge and Skills
Strong understanding of hospice philosophy, end-of-life care, and family dynamics.
Excellent clinical assessment, documentation, and communication skills.
Ability to apply knowledge of community resources and interdisciplinary care.
Skilled in regulatory compliance, including Medicare and Medicaid guidelines.
Demonstrated empathy, flexibility, and professionalism in sensitive patient situations.
Working Conditions
Patient homes, inpatient facilities, or remote triage environment.
Potential exposure to infectious materials in the field.
May require occasional travel for patient visits, training, or team meetings.
