Overview JOB SPECIFICS Overview - Palliative Care Collaborate with 1-3 physicians and 3 advanced practice providers, case manager, hospice coordinator, and Pastoral Care Team members Responsibilities include assessing a variety of symptom management cases, promoting communication and collaboration of multiple disciplines for end-of-life care and providing passionate care for patients with chronic and life-limiting conditions. Patient Assessment and Diagnosis: Conducting comprehensive physical exams and patient histories. Ordering and interpreting diagnostic tests (labs, imaging, etc.). Diagnosing common medical conditions and diseases. Patient Assessment and Diagnosis: Treatment and Management: Developing and implementing treatment plans. Prescribing medications and therapies. Monitoring patient progress and adjusting treatment plans as needed. Managing chronic conditions such as diabetes, hypertension, and asthma. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement.
