Position Highlights:

Schedule: 7:30AM - 5:00PM 4 days per week patient-facing, with one half-day dedicated to administrative time (remainder of the day off).Provide comprehensive/primary care to chronically ill patients in an office settingAverage of 14 patients per dayFocus on reducing hospital readmission rates through proactive patient management and educationConduct thorough assessments, diagnose conditions, and prescribe appropriate treatmentsRobust APP onboarding program consistent with didactic curriculum, specialty shadowing, competency-based evaluations, and mentorship
