This role provides advanced nursing care in an acute care setting, focusing on a specialized patient population. The APN will work both independently and collaboratively with physicians, case managers, and the healthcare team to ensure comprehensive, high-quality care. **Key Responsibilities:** + Deliver expert care to heart failure patients, promoting healing and preventing illness and hospital readmission. + Assess patients’ health status through comprehensive history-taking, physical exams, and psychosocial evaluations. + Develop, implement, and modify individualized treatment plans based on patient needs and clinical data. + Perform specialized procedures and interventions within the scope of practice. + Coordinate care transitions across service units to optimize patient outcomes. + Educate patients and families, empowering them to participate in their healthcare decisions. + Maintain high standards of practice in alignment with Loyola University Medical Center’s mission and care models. **What we offer:** •    Benefits from Day One •    PTO & CME Allocation •    Malpractice Insurance (Incl.
