**What you’ll do:**
Loyola University Medical Center is seeking a skilled and compassionate **Advanced Practice Nurse (APN)** to join our **Heart Failure Program** . This role provides advanced nursing care in an acute care setting, focusing on a specialized patient population. The APN will work both independently and collaboratively with physicians, case managers, and the healthcare team to ensure comprehensive, high-quality care.
**Key Responsibilities:**
+ Deliver expert care to heart failure patients, promoting healing and preventing illness and hospital readmission.
+ Assess patients’ health status through comprehensive history-taking, physical exams, and psychosocial evaluations.
+ Develop, implement, and modify individualized treatment plans based on patient needs and clinical data.
+ Perform specialized procedures and interventions within the scope of practice.
+ Coordinate care transitions across service units to optimize patient outcomes.
+ Educate patients and families, empowering them to participate in their healthcare decisions.
+ Maintain high standards of practice in alignment with Loyola University Medical Center’s mission and care models.

**What you’ll need for this job:** ** ** ** **
+ CPR – Basic Life Support
+ ACLS – Advanced Cardiovascular Life Support
+ Nursing – Current Registered Nurse License - State of Illinois (RN)
+ Nursing – Nursing Practitioner License – State of Illinois (APN)
+ Physician Assistant License – State of Illinois (PA)
Pay Range: $109,304 - $180,336 per year
_Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles._
